ㅤContext to the study:
Can you tell me about a study that compares HoLEP to TURP for bladder outflow obstruction?
ㅤ✅ Take-home message of study:
HoLEP is non-inferior to TURP for relief of bladder outflow obstruction, with fewer re-operations at 7 years follow up
ㅤ Randomised Controlled Trial
ㅤ
Study participants:
31 (14 HoLEP and 17 TURP) of an initial 61 patients (18 lost to follow up, 12 deceased)
'urodynamically obstructed'
prostate volumes between 40 and 200 mL
Excluded patients with urinary retention
ㅤ
ㅤ
Key study outcomes:
Confirmed that HoLEP is at least equivalent to TURP in the long term
On average, participants who underwent both TURP and HOLEP went from having severe lower urinary tract symptoms at baseline, to mild symptoms with durable results
No statistically significant difference in AUA symptom score, QOL, Qmax, continence or erectile function for TURP Vs HoLEP up to 7 years follow up
HoLEP had superior perioperative outcomes: reduced morbidity, catheter time and hospital stay
No patients in HoLEP arm required re-operation, 3 required re-operation in the TURP arm
ㅤ
ㅤ
Study Limitations:
Small numbers at 7 years follow up -> limits power incl. of sub-group analysis
18 lost to follow up, 12 deceased
Results are not generalisable to patients with urinary retention
